I hereby grant to M.P.S. the absolute and irrevocable right and permission in respect of photographic portraits or pictures that has been taken of me, to copyright the same, in his own name or otherwise; to use, reuse, publish and republish the same in whole or in part, individually or in any or in all media now or hereafter known, for the strict purpose of self promotion by way of exhibition in a public gallery, art, photographic or other; to publish as a personal folio or instructional material in photographic magazines and to use the entities name in connection therewith.

I am of full age and have the right to contract in my own name. I have read the foregoing and fully understand the contents thereof.

I consent to the inclusion of this agreement including the personal details recommended in it being added to the public database owned or operated by YOUR COMPANY and or any entity acting under the permission of YOUR COMPANY I acknowledge that there is no fee payable by myself to YOUR COMPANY and there is no fee payable to me by YOUR COMPANY.
